Confirmation Statements

The Open Enrollment period has closed, and your 2022 benefits confirmation statement is now available for review online.

Your 2022 benefits confirmation statement summarizes the following:

  • Your 2022 benefits elections.

  • Live Well credits. The statement will reflect any credits applied to your monthly premiums if you completed the Live Well Rewards program from November 15, 2020 through November 15, 2021 and submitted the online form.

You may access your 2022 benefits confirmation statement by:

  • Logging in here from your computer or tablet
    OR

  • Logging in here from your mobile device

To make corrections to your benefit elections, you must take the following steps by Monday, Nov. 29:

  • View your 2022 benefits confirmation statement using the links above.

  • If viewing the computer or tablet version, click on the prompt above your listed benefits to email askHR.

  • If viewing the mobile version, email askHR@nd.edu directly and identify the desired correction.

  • Specific changes to benefits must be sent no later than Monday, Nov. 29.

  • If you have any difficulties with the link, you may send a message directly to askHR@nd.edu.

A few things to keep in mind regarding health care spending accounts and 2022 benefits enrollment: 

  • If you elected the HSA Plan for 2022 and have a flexible spending account (FSA) for 2021, you must receive full reimbursement by December 31, 2021 or you will forfeit the balance.
  • If you would like to adjust your FSA contribution, please follow the instructions on how to make corrections to your benefits elections above. The IRS has recently increased the maximum amount that may be contributed to a health care spending account from $2,750 to $2,850.
  • This is your last opportunity to enroll in benefits for 2022.
