Notice Regarding Wellness Program

Notre Dame’s new Live Well Rewards program provides a credit to your 2022 health plan premium, essentially lowering what you pay for health coverage next year. (Note: The Live Well Rewards program replaces the previous health screening/premium credit program that has been used for several years.)

We care about your total well-being, so we’ve designed the Live Well Rewards to focus on holistic well-being and preventive care. We’ll reward you for completing any (or all!) of the following:

REWARD AMOUNTS & PREMIUM CREDIT

The more activities you complete, the higher the well-being incentive you earn:

Wb Incentive Checks

Your Live Well Rewards will be applied to your 2022 health plan premium. The credit will be evenly divided throughout the year. For example, if you complete three activities and earn $360, the credit will be divided by 12 months—lowering your premium by $30 each month.

TIME PERIOD & ELIGIBILITY

  • Time period to complete activities: November 15, 2020, through November 15, 2021
  • Eligibility: Benefit-eligible faculty and staff only
    Spouses are not eligible to participate. If you complete just two activities, you will earn more than your combined employee/spouse incentive in previous years.

HOW TO PARTICIPATE

To participate, fill out the online Live Well Rewards form to indicate the activities you’ve completed. You are required to submit all activities at once via the online form. When you have all activities entered that you want to include, then hit “submit” to finalize. (Note: You cannot submit the form each time you complete an activity.)
