Do I need to notify HR of a departmental name change?
If your department is part of the campus administration (non-Academia), contact the Office of Human Resources with the new information. If your department is part of Academia, you must contact the Office of the Provost to report the change.
The Department Name Change form is used to make changes to the Database. When using this form, keep in mind the following guidelines:
There are three versions of the name that we need: a 40-character name, a 30-character name, and a 16-character name. These numbers are the maximum allowable characters for the fields. You must count spaces as well.
Example
Office of University Human Resources - 40 characters
maximum (This example 36)
Office of Human Resources - 30 characters maximum
(This example 25)
Human Resources - 16 characters maximum (This example
15)
Complete the Department Name Change portion of the form and send the form to the Human Resources Information Technology Department, 200 Grace Hall or Fax the form to 631-6862.
