What forms need to be submitted for a new hire?
Before a candidate may begin employment, the forms listed below must be completed. Please submit these forms to Office of Human Resources, 200 Grace Hall. No one should begin work until these forms have been completed.
- Staff Personnel Action Form (Staff PAF)
- Employment Eligibility (I-9); (Please be sure to include copies of appropriate identification to prove identity and work eligibility) Directions for Completing the I-9.
- Tax Forms (Both federal W-4 and state W-4 Forms can be found on the HR Forms Page)
