What are the preventative care benefits?

Your benefits are as follows:

  • Physical exam
  • Well Woman Care (includes PAP smear for teenage females)
  • Mammogram
  • Blood screening
  • Sigmoidoscopy
  • Occult blood
  • Prostrate-Specific Antigen
  • Eligible immunizations: DPT, MM, Tuberculin skin test and annual flu shot.

No copayment is required for preventative care.  Refer to the plan document for age and frequency rules.
