How do I apply for COBRA insurance?
When a qualifying event occurs you will receive a COBRA notice from Meritain Health at your home address. This notice gives you the option to elect coverage or decline coverage. If you elect coverage you would send the election form back to Meritain Health. At that time you would be reinstated with the health, dental, and/or vision insurance elected.
