If you are a 2015 Notre Dame health benefit plan participant:
Beginning early next week, Notre Dame faculty and staff who were enrolled in a University health plan during 2015 will be mailed a Form 1095-C.
This new form is an IRS requirement under the Affordable Care Act. Much like your W-2 or 1099 reports your income, Form 1095-C reports the health care coverage you had or were offered in 2015.
You should not need this form to file your taxes. If you have already filed your taxes, you should not need to refile. Form 1095-C is generally only filed by individuals seeking subsidies on the health care exchange, which enrollees in University health plans typically do not require.
