The 2014 Open Enrollment period has closed, and your 2014 Benefits Confirmation Statement is now available for review online at:
The statement summarizes your 2014 elections. The elections you made to your medical, dental, vision, flexible spending account, and life insurance benefits during the Open Enrollment period are reflected in the statement. If you did not make any elections during the Open Enrollment period, the statement summarizes those benefits that were automatically carried over from 2013. Note: flexible spending account elections do not automatically carry over.
The statement also reflects any credits applied to the monthly premiums of eligible faculty, staff and spouses who completed a biometric screening and Your HRQ.
Statements are available online. To access your statement, visit openenroll.nd.edu. Or you may log in to inside.nd.edu, click the “My Resources” tab, look for the Human Resources panel, and click “Open Enrollment.”
To make corrections or changes to your benefit elections,please print your confirmation statement, indicate changes on the form, sign and date the form, and return it to the Office of Human Resources at 200 Grace Hall. Changes must be received by Friday, December 6th. If you require no changes, no action is needed.
