Policy

The University offers a flexible Group Benefit Plan, which includes options in medical insurance, dental insurance, life and dependent life insurance, vision insurance and flexible spending accounts. The Group Benefit Plan is designed to allow eligible employees the opportunity to choose benefit coverage that best suits their needs. Benefit premiums through payroll deduction are made with pre-tax dollars which provides significant tax savings.

Eligibility

Full-time administrators, faculty, and staff members are eligible to participate in the Group Benefit Plan. Full-time is defined as working at least 30 hours per week during the calendar year (for 12 months). Coverage is effective the first day of the month on or after the date of hire, or date of status change. Employees not eligible to participate in the full Group Benefit Plan program may elect to participate in specific parts of the program. Benefit Summary Sheets provide specific information on benefits eligibility by employment category.

Employees who terminate insurance coverage are issued a Certificate of Creditable Coverage when they are notified of their eligibility to continue insurance coverage through COBRA.

Enrollment

New employees are offered the opportunity to enroll in the Group Benefit Plan during the New Employee Orientation. Administrators, faculty, and staff who do not complete the necessary enrollment forms within 60 days are automatically enrolled in the PPO health plan with individual coverage and in basic life insurance coverage. Subsequent changes can be made during the next annual open enrollment period.

Changes

An open enrollment period is scheduled annually to give employees participating in the Group Benefit Plan the opportunity to review their benefits and make changes as necessary. In accordance with IRS regulations, changes outside the open enrollment period are allowed only when the employee experiences a qualifying life event change. In these situations, an employee has 31 days from the life event change to notify the Office of Human Resources. Please see the information on qualifying life events.

Group Benefit Plan Options

Medical/Prescription Drug Insurance – Full-time administrators, faculty, and staff may select a medical insurance plan from several options offered or may waive medical coverage by providing proof of insurance from another source. Premiums are based on contributions from the University and the employee with the actual premium amount determined by the plan selected. Part-time administrators, faculty, and staff also have the option to select a medical insurance plan with the full cost of premiums being paid by the employee. Currently there are three insurance plans offered. A PPO, an HMO and a High Deductible Health Plan (HDHP). Plans are reviewed annually to ensure continued cost effectiveness and value, to provide for continuation of comprehensive benefit coverage, and to assure continued inclusion of a comprehensive group of health care providers and facilities. Enrollment in one of these medical plans automatically includes enrollment in the University offered Prescription Drug plan.

Dental Insurance – Participation in a dental plan is optional. Full-time and part-time administrators, faculty, and staff may select from one of two dental plans offered.

Vision Insurance – Participation in the vision insurance plan offered is optional for full-time and part-time administrators, faculty, and staff.

Life Insurance – Full-time administrators, faculty and staff are automatically provided a $25,000 group term life insurance policy at no cost. Optional additional life insurance may be purchased in increments from 1 time up to 10 times annual salary.

Dependent Life Insurance – Full-time administrators, faculty, and staff are offered the option of purchasing Dependent Life Insurance coverage for their spouse, unmarried dependent children younger than 19, and any unmarried children between the ages of 19 and 25 who are full time students and still considered dependents. Flexible Spending Accounts – Full-time administrators, faculty, and staff are offered the option to enroll in two flexible spending accounts:

  • Health Care Spending Account
  • Dependent Care Spending Account

These spending accounts allow employees to set aside pre-tax dollars to pay for eligible health care and dependent care expenses that arise throughout the year.
