Anthem Blue Traditional $250 (Medical Service)
This plan provides coverage after Medicare has made its appropriate payments. Plan participants must pay a $250 deductible for Major Medical benefits (if Medicare does not cover an expense and/or Medicare benefits are exhausted). After the deductible, the plan pays 80% of usual and customary charges for covered medical expenses. The maximum out-of-pocket limit is $2500 for an individual in a calendar year. After that, the plan pays 100% of the usual and customary charges for covered medical expenses for the remainder of the calendar year. All covered services under Major Medical must be determined by Anthem to be medically necessary.
